Medical Case Managers

The purpose of the Medical Case Manager is to provide medical support, care, and education to the residents of The Little Tree Project Residential Program. They will be responsible for providing routine care, observing patients' health, and communicating instructions to patients regarding medication, home-based care, and preventative lifestyle changes. 

As a retired or practicing health care professional, you can help us in many ways at Little Tree. Until the organization is able to open its own health clinic, we will partner with local health care facilities and providers in helping survivors receive the best health care possible. 

In the meantime, Little Tree will need medical case managers and advocates. A medical case manager will be responsible for the overall health care of the survivors in the care of the organization. Medical Advocates will be volunteers who have experience in the medical field as well as trauma and are willing to escort survivors and support them in doctor appointments and medical procedures. 

When the Little Tree Project trauma-informed health clinic is opened, the organization will need volunteer nurses, doctors, nursing technicians, counselors and therapists until it receives funding necessary to hire those positions.

Goals:

  • Perform an initial physical health analysis of the new client and determine what is needed. Set client up with medical care as appropriate for age, as well as mental health care, dental care, and and eye exams 

  • Provide follow up as needed for the time in program.

  • Offer education for clients in either individual or group settings regarding basic wellness or disease specific education as needed

  • Consult with staff and external agencies to provide support services and resources (medical offices, hospitals, pharmacies, dental offices etc.)

  •  Keep comprehensive records of clients’ progress throughout the process, including phone calls, and pertinent discussions

  • Meet with women before and after appointments to discuss appropriate questions to ask providers, and then follow up after to ensure the services were beneficial and understood

  • Attend appointments with women or provide another nurse to go as necessary

  • Help with financial paperwork and prescription assistance paperwork as needed

  • Monitor blood pressure, weight, glucose as needed

  • Triage as necessary regarding if home care, ER care or an office visit is warranted

  • Conducts regular random drug testing

A medical case Manager can also have the duties as a Prescription Manager, Wellness Group Manager, and/or Health Educator.

Prescription Manager:

  • Work with women individually and staff as necessary to maintain compliance and education in all aspects of prescription medications.  

  • Keep weekly med boxes filled, allowing the women to demonstrate that they know how as well on a bi-monthly basis

  • Monitor medication compliance and Medical Case Manager/Residential Support Manager updated

  • Keep a variety of over the counter medications on hand and sample RX meds (only given if they have RX for exact same med) for the women as needed.

  • Manage the calendar for RX pick-ups, support for appointment calendar

  • Ensure women don’t run out of medications

  • All routine meds are picked up 3x/month 


Wellness Group Manager: coordinate and/or lead 

  • Walking groups in the mornings and M/W/F at noon, yoga when possible

  • Work with outside agencies to provide shoes and workout gear for women 

  • Weekly nutrition, cooking camp

  •  Nutrition classes monthly (nutrition that is affordable on food stamps)

  • Plan and organize 2 5K walks/year.  

  • Lead ALA Freedom from Smoking class 

  • Recruit pro-bono Health Coaches

  • Organize yearly dinner, and nurses to come give an in depth presentation on liver disease


Health Educator 

  • Speak to Nursing and Medical school students regarding ACE Scores, Childhood trauma resulting in substance abuse, prostitution and chronic health conditions.  

  • Barriers to access to care for uninsured and under-insured clients, including stigmas, and lack of health literacy etc.  

  • Speak at Education workshops in order to help facilitate new programs starting out, with a focus on Health and Wellness


Aftercare Clinical Therapists/Counselors

A Counselor is a qualified individual who uses counseling methods to help people manage and overcome mental and emotional issues. Their duties include listening to patients, developing treatment plans, and creating coping strategies. 

Just like a healthcare professional, if you are a retired or practicing mental health care professional, we would love to work with you in serving our clients! 

A trauma-informed therapist is aware of the complex impact of trauma on a person’s suffering and how it shapes a person’s efforts to cope. A trauma-informed approach integrates a thorough knowledge of this impact into every aspect of treatment. This means that Little Tree Project and any counselor serving our clients makes emotional and psychological safety a priority.

Assists the organization in providing pro-bono general and specialized health care to survivors of human trafficking.
